Job Description
CAN Community Health is hiring a Medical Case Manager II to coordinate and deliver comprehensive case management services to clients with HIV, Hepatitis C, STI's, and other infectious diseases. This role involves eligibility screening, care planning, and collaboration with healthcare teams to promote optimal health outcomes.
Requirements
- Bachelor’s degree in social work, Nursing, Public Health, or related field or 4/7 years’ experience in same fields.
- Three years of HIV prevention/intervention experience
- MCM Certification for TGA, or able to obtain within 90 days
- Valid driver’s license and insurance
