Nurse Practitioner - HART Hub Brampton

The Canadian Mental Health Association - Peel Dufferin Branch is seeking a part-time Nurse Practitioner to join the HART Hub Brampton team. The Nurse Practitioner will work closely with the Rapid Access Addiction Medicine (RAAM) team and assess and treat symptoms of substance use and facilitate connections to continuum of care to increase access to additional community-based services to address social determinants of health.

Requirements

• Masters of Science in Nursing, and Registered Nurse Practitioner (RN-EC) in good standing with the College of Nurses of Ontario
• Minimum 2 years of experience in Mental Health and/or Addictions in a community setting
• Completed College Approved Controlled Substances Education Program
• Psychiatric Nursing Certification is an asset
• Fluency in a second language, including ASL (American Sign Language) is considered an asset
