Nurse Practitioner position at the Sudbury Mental Health and Addictions Centre, providing client-centered care in a multidisciplinary team.
Requirements
- Practice autonomously as a Nurse Practitioner
- Perform health assessments and provide evidence-based treatment for clients with addictions and concurrent disorders
- Communicate with clients about health assessment findings and diagnosis
- Consult and refer to members of the multidisciplinary team and community services
- Provide direct comprehensive care to clients managing acute and chronic addictions
- Counsel clients on harm reduction, withdrawal, and craving symptom management
- Develop and maintain professional working relationships with team members and community agencies
- Assess population health trends and patterns and participate in designing services
- Develop and implement standards, guidelines, policies and procedures for addictions services
- Participate in evaluation and research studies related to withdrawal management services
- Attend professional council meetings and sit on various committees and meetings
- Determine and align improvement projects with HSN's Strategic Plan
- Evaluate patient care and satisfaction and formulate solutions for improvement
