RN Case Manager

Care Dimensions is seeking a RN Case Manager to provide compassionate care to patients and families facing serious illness. The role involves coordinating patient care, developing comprehensive care plans, and advocating for patients and families.

Requirements

• Active Massachusetts RN license in good standing
• Associate’s degree in Nursing required; BSN or equivalent preferred
• Proven ability to provide care with compassion and cultural sensitivity in diverse home or facility settings
• Strong interpersonal skills and the ability to thrive in a collaborative team environment
• Flexibility, self-direction, and sound clinical judgment
• Valid driver’s license and access to an insured, reliable vehicle
