Care Plan Coordinator PACE

Colorado PACE has an immediate opening for a Care Plan Coordinator. The position is part-time, with a schedule of Monday-Friday, 24-28 hours per week. The hourly pay range is $25.29 - $29.71, with supplemental pay for overtime. The position requires a valid Colorado driver's license and proof of auto insurance. The Care Plan Coordinator will manage the schedule, logistics, and documentation for all initial assessments, periodic reassessments, and annual care plan reviews of participants by the IDT. They will also ensure compliance with PACE program regulations and standards of care, and maintain accurate and up-to-date data within the electronic medical record system.

Requirements

• Associate's degree in healthcare, administration, or a related field
• One or more years of experience in care coordination, case management, or a highly administrative role within a healthcare setting
• Valid Colorado Driver's License and proof of auto insurance
• LPN/Bachelors degree preferred
