To provide support to service users in line with the organisation’s aim to provide a high quality, reliable support service tailored to suit each of our service users individual support plans or personal care plans and contribute to the safeguarding of those receiving care whilst respecting their rights to be autonomous in their decision-making.
Requirements
- Deliver planned and allocated care in a flexible manner, across a variety of health and social care settings as directed by a senior member of the team developing own competencies to meet the demands of the service required.
- Provide personal care in accordance with the service users personal care plan to facilitate them in reaching their full potential which may involve assistance with; continence requirements; health and medication requirements; manual handling; eating and preparing meals; handling personal possessions; entering the home, room, bathroom, toilet, and shopping.
- Support services users’ social needs in areas such as travel training, money management, and independent life skills.
- Ensure provision of a safe environment to support services users in their chosen activities supporting them to manage their individual domestic and personal resources.
- Actively listen to service users and/or relatives' concerns providing clear feedback and referring to other members of the team as required.
- Treat service users with respect, enabling them to make their own choices by obtaining informed consent for any care or support that you provide and acknowledging their right to decline assistance.
- Ensure accurate and appropriate care plan record keeping at all times.
- Observe and maintain utmost confidentiality is always maintained both within and outside of the workplace.
- Ensure understanding and awareness of any documented advanced plans that are in place for individuals and identify and instigate emergency procedures such as Basic Life Support where required.
- Promote effective communication and relationships with service users, work colleagues, and all other health and social care workers whilst being aware of communication differences and adopt an approach that minimises the effect of such differences.
- Adhere to and promote high standards of infection prevention and control at all times including effective hand and surface cleansing to appropriate use of and disposal of Personal Protective Equipment (PPE).
- Promote all health, safety, and security measures within a services user’s home environment and/or other care providers where he/she is deployed.
- Ensure all incidents are reported and recorded appropriately, accurately, immediately.
- Adhere to and comply with all company policies and procedures eg Data Protection, Social Media, Appearance, Diversity & Inclusion.
- Demonstrate an awareness of costs in terms of supplies, equipment and anything within a service user own home requiring use to deliver care.
- Work safely whilst being aware of need to avoid waste which may impact on the service user and/or the employing company.
- Ensure accuracy and timeliness of timesheet submission to facilitate effective financial administrative processes by submitting timesheets in a timely manner.
- Where you are required to drive as an essential element of the role, ensure the routes taken are as direct as possible.
- Demonstrate commitment to achieving a health and social care level 2 vocational portfolio of evidence to develop knowledge, skills and qualifications for the Support Worker/HCA roles.
- Undertake any mandatory training as directed by Care Hub in addition to any development needs identified at appraisal.
- Attend and contribute to supervision sessions to support own development and regulatory standards.
