Job Description
The CHAPLAIN acts as a community liaison, connecting patients with local parishes or synagogues, providing spiritual counseling, and coordinating service within the team. This role involves documentation, administrative tasks, and communication with the interdisciplinary team to ensure the patient's well-being. The position is full-time, Monday-Friday, with a shift of 8:00 AM to 5:00 PM.
Requirements
- Assess spiritual factors related to patient care and intervene appropriately.
- Establish a spiritual care plan for patient / families.
- Provide accurate and appropriate documentation.
- Coordinate spiritual services within the team structure.
- Administer sacraments as requested and perform funeral or memorial services.
- Provide spiritual support, individual or group basis.
- Maintain communication with the interdisciplinary team.
- Maintain a commitment to clinical or pastoral education.
