Job Description
Catholic Hospice Inc. is seeking a Part-Time Registered Nurse to provide direct patient care in a visiting and intermittent basis, in the patient’s home. The role involves conducting assessments, developing care plans, and collaborating with the clinical and administrative teams to ensure high-quality patient care.
Requirements
- Complete initial and ongoing assessments
- Perform regular assessments at designated times
- Develop, initiate, and revise nursing care plans
- Coordinate services and communicate changes
- Supervise other nursing personnel
- Maintain documentation and participate in in-service programs
