Job Description
The Patient Navigator RN assists patients in understanding their diagnosis, coordinates treatment plans, and communicates with providers and staff. Responsibilities include guiding patients and families through their healthcare experience, following patients from diagnosis to completion of care-path, and maintaining professional and technical knowledge.
Requirements
- Graduate of an accredited nursing program required. BSN preferred.
- 2 years' previous clinical experience.
- RN License and certification as required by state.
- BLS certification required.
