Job Description
The Case Manager will facilitate the improved wellbeing of individuals with severe mental illness and/or substance use disorders by developing and implementing treatment plans, providing psychosocial rehabilitation, and coordinating community resources.
Requirements
- Builds therapeutic rapport with clients
- Provides psychosocial rehabilitation and symptom monitoring
- Maintains a treatment-oriented environment
- Coordinates utilization of community resources
- Provides crisis intervention and supportive counseling
- Assists clients in financial planning and housing
- Formulates discharge plan
- Meets department-designated billing quotas
