Clinical Pharmacist Lead

We're looking for a Clinical Pharmacist Lead to build and lead our medication management function across GDMT and broader population health programs. This is a unique opportunity for a strong operator who starts by directly managing patients, shaping workflows, and defining best practices, and evolves into building and scaling a high-performing pharmacy team.

Requirements

• PharmD required; active pharmacist license in DC, MD, VA, GA, TX
• 5–8+ years of clinical pharmacy experience, ideally in cardiology, ambulatory care, or population health
• Experience with heart failure management and chronic disease management
• Prior experience building or scaling programs
• Demonstrated ability to move from individual contributor to people leader
• Experience in value-based care, population health, or managed care preferred
