As a Mobile Case Manager with Chartiers Center, you would be working in the community to provide support and services to individuals with mental health challenges.
Requirements
- Coordinate and oversee the delivery of case management services to ensure that the individuals are connected to various benefits and services.
- Assess individual’s needs and develop individualized service plans to meet those needs.
- Maintain regular communication with clients to monitor progress and address any concerns.
- Collaborate with other service providers and/or administrative staff to ensure seamless delivery of services.
- Maintain strong community relationships with contacts for education, employment, housing and natural supports.
- Maintain accurate records of interactions, service plans, and progress reports.
