The Blended Case Manager is responsible for developing, coordinating, and implementing services for consumers as identified in the service plan, providing case management, service planning, and coordination of services with other agency providers to meet the client’s needs along with supportive counseling.
Requirements
- 12 credits in coursework related to Human Services, Psychology or related field
- One or more years of direct clinical experience working with children, youth and families
- Manage a varied work schedule Monday-Friday between 8:30am - 8:30pm, including 3 required evenings per week
