Acute Care RN Case Manager

We're seeking an Acute Care RN Case Manager to join our team. As a case manager, you will be responsible for achieving positive patient outcomes and managing quality of care across the continuum of care. You will work closely with other members of the care team to develop effective plans of care and high levels of care coordination.
