RN Case Manager

We're looking for a Nurse Case Manager to join our team. As a case manager, you will be responsible for managing patient outcomes and coordinating care across the continuum of care. You will work closely with other members of the care team to develop effective plans of care and coordinate transitions of care. This role requires strong interpersonal and communication skills, critical thinking, and the ability to work autonomously.

Requirements

• Bachelor's degree in Nursing
• 2+ years of experience in case management or a related field
• Strong interpersonal and communication skills
• Critical thinking and problem-solving skills
• Ability to work autonomously
• Knowledge of nursing and case management theory and practice
• Knowledge of patient care charts and patient histories
• Knowledge of clinical and social services documentation procedures and standards
