We're a unique primary care provider rapidly expanding and seeking a Care Facilitator to join our team. As a patient advocate, you'll deliver exceptional customer service and provide basic secretarial and administrative support.

Requirements

• High school diploma or equivalent education required
• A minimum of 1 year of work experience in a medical clinic or similar environment required
• BLS for Healthcare Providers preferred
