Primary Care Physician

We’re looking for a Primary Care Physician to join our team. As a PCP, you will provide care for patients with acute and chronic illnesses, lead your care team, and help champion a positive culture in our medical centers. Requirements

• Provide care for patients with acute and chronic illnesses
• Lead care teams and help champion a positive culture
• Demonstrate accountability for outcomes, strong clinical care, and cost-effectiveness
• Act as a health coach to help patients set and achieve health goals
• Simplify and prioritize care to make behavior change more actionable
• Meet departmental goals and objectives, and regulatory compliance
• Engage with hospitalists and transitional care teams as needed
• Take an active role in the management of the medical center and recruiting patients and providers
