Job Description
We're looking for a Primary Care Physician to join our team at ChenMed, a physician-led and mission-driven primary care organization. As a PCP, you'll provide care for patients with acute and chronic illnesses, work closely with other healthcare professionals, and help champion a positive culture of love, accountability, and passion within our centers.
Requirements
- Provide care for patients with acute and chronic illnesses
- Work closely with other healthcare professionals
- Help champion a positive culture within our centers
- Be accountable for patient outcomes and cost-effectiveness
- Act as a health coach to help patients set and work towards health goals
- Simplify and prioritize care to make behavior change more actionable
- Lead care teams and manage patient care coordination
- Supervise and collaborate with Advanced Practice Practitioners (APPs)
- Be available and accessible to patients, provide evidence-based medicine, and develop leadership skills
