Community Health Plan of Washington is seeking a Case Manager I to provide case management services for pediatric, adolescent, and perinatal members. The Case Manager I will be responsible for assessing, evaluating, and planning care for members, as well as coordinating with healthcare providers and community resources to achieve positive health outcomes.
Requirements
- Bachelor's degree in nursing or a related field
- Current, unrestricted license in the State of Washington as a registered nurse (RN) or social worker (LSWAA, LSWAIC)
- Minimum of 1 year of case management, home health, or discharge planning experience
- Minimum of 3 years of clinical experience in an acute care and/or outpatient setting
- Experience with Microsoft Office products
- Case Management Certification (preferred)
- Bilingual abilities (preferred)
