Requirements
- Assesses the patient initially no greater than 48 hours of admission order and collaborates with other Interdisciplinary Group disciplines toward the Comprehensive Assessment within 5 calendar days
- Maintains regular communication with the Patient Care Manager for Hospice Services to review caseload
- Maintains up-to-date patient records so that problems, plans, actions, and goals are accurately and clearly stated
