Requirements

• Assesses the patient/family caseload
• Develops a comprehensive plan of care and goals
• Provides Hospice Services nursing care based on systematic assessment
• Maintains accurate and up-to-date patient records
• Communicates with attending physicians, other Hospice staff members, and other Hospice agencies
