Registered Nurse for Hospice Services

Join us at Affinis Hospice, a non-profit, mission-driven organization that's known for the highest level of care in our communities. We're looking for a Registered Nurse to provide professional nursing care to Hospice Services patients.

Requirements

• Responsible for identifying patient/family needs and for providing supportive care in accordance with the attending physician's orders, plan of care and the Hospice Services policies and procedures.
• Assesses the patient initially no greater than 48 hours of admission order and collaborates with the other Interdisciplinary Group disciplines and others as needed toward the Comprehensive Assessment within 5 calendar days of the election of the patient for Hospice Care.
• Maintains regular communication with the attending physician concerning Hospice Services patient/family caseload.
