Registered Nurse for Hospice Services

As a Registered Nurse, you will be responsible for providing professional nursing care to Hospice Services patients, identifying patient/family needs, and providing supportive care in accordance with the attending physician's orders, plan of care, and Hospice Services policies and procedures.

Requirements

• Assumes primary responsibility for a Hospice Services patient/family caseload
• Assesses the patient initially no greater than 48 hours of admission order
• Maintains regular communication with the Patient Care Manager for Hospice Services
• Collaborates with other members of the Interdisciplinary Group to develop a comprehensive plan of care
