Job Description
We are looking for an HIV Medical Case Manager to provide medical and supportive case management services to people living with HIV. The ideal candidate has a bachelor's degree in a related field and 2 years of experience in case management, preferably in a clinic setting.
Requirements
- Bachelor’s degree or equivalent in social work, sociology, public health, or related field
- Two years of experience in case management in a clinic setting or 5 years in a non-clinical setting
- Experience working with patients with mental health diagnoses, substance use disorders, and/or complex social or medical needs
