Occupational Therapist consistent PRN

Vertis Therapy is seeking a dedicated PRN Occupational Therapist for their skilled nursing facilities in Ft Wayne, IN. The role involves providing ongoing support and training to staff, managing patient treatment through therapeutic procedures, and coordinating with the interdisciplinary team. This position focuses on quality patient care and utilizes a therapist-owned company model.

Requirements

• Review available patient information and evaluate the impact of this information on the assessment and treatment process.
• Conduct and document a thorough functional assessment of the patient.
• Develop and implement an individualized treatment plan based on patient needs.
• Record patient progress in the patient record a minimum of every seven (7) days.
• Revise treatment plans, as needed, throughout the treatment process.
• Instruct Assistants in treatment procedures and provide close supervision.
• Perform patient care tasks, including exercises, monitoring, and referrals.
• Access continuing education opportunities.
• Participate in quality improvement processes.
• Perform other duties as assigned.
