Job Description
Vertis Therapy is seeking a passionate and results-oriented Occupational Therapist to provide consistent PRN hours at their assisted living facility in Elkhart, IN. This role emphasizes providing quality, patient-centered care through therapeutic procedures to improve independence, safety, and quality of life. The therapist will evaluate patient needs, develop individualized treatment plans, and collaborate with a multidisciplinary team.
Requirements
- Review available patient information
- Conduct and document functional assessments
- Develop and implement individualized treatment plans
- Comply with evaluation, treatment and documentation guidelines
- Record patient progress
- Revise treatment plans
- Document the course of treatment
- Instruct assistants in treatment procedures
- Orient patients to program services and treatment procedures
- Assist in the referral process
- Participate in team meetings
- Maintain current knowledge of community resources
- Attest to patient success
