Job Description
Vertis Therapy is seeking a passionate Physical Therapist to provide consistent PRN hours at their Assisted Living facility in Elkhart, IN. The role involves evaluating and treating patients with personalized care, focusing on functional impairment and independence. The therapist will participate in interdisciplinary teamwork and document patient progress.
Requirements
- Review patient information and evaluate impact on assessment and treatment.
- Develop and implement individualized treatment plans.
- Record patient progress and adhere to documentation guidelines.
- Revise treatment plans as needed and provide clinical instructions.
- Assist patients with discharge planning and community resources.
