Job Description
Care Coordinator - Duffield, VA: Crossroads Treatment Centers is seeking a Care Coordinator to assist in the facilitation of recovery services and ensure proper documentation in patient records. The Care Coordinator will work with a buprenorphine-waivered practitioner and utilize collateral contacts to provide services not offered by the program.
Requirements
- Bachelor's degree in social work, Psychology, Psychiatric Rehabilitation, Sociology, Counseling, Vocational Rehabilitation, or Human Services Counseling
- At least one year of experience providing services to individuals with a diagnosis of substance use disorders
- Licensure as a Registered Nurse with at least one year of direct experience providing services to individuals diagnosed with a substance use disorder
- Board of Counseling Certified Substance Abuse Counselor (CSAC) or CSAC-Assistant under supervision of a CSAC
