- One year of experience in mental health, hospital, medical outpatient clinic, FQHC, Home Health or long-term care setting
- Master’s Degree/Licenses, Certifications, Registrations
- Current unrestricted license in the state in which practicing is required upon hire
- Basic Life Support (BLS) for Health Care Providers
- National Provider Identifier (NPI) required prior to employment start date
