Job Description
LCSW/LPCC/LMFT provides comprehensive psychosocial assessments and behavioral health care. Services are designed to support and maximize the client’s ability to better direct their own health care experiences.
Requirements
- One (1) year of experience in mental health, hospital, medical outpatient clinic, FQHC, Home Health or long-term care setting
- Three to five (3-5) years of experience in related environment
- Master’s Degree/Licenses, Certifications, Registrations
- Current unrestricted license in the state in which practicing is required upon hire
- Basic Life Support (BLS) for Health Care Providers
- National Provider Identifier (NPI) required prior to employment start date
