Care Manager Extender

Care Manager Extender provides care management functions, documentation, referral and linkage, and monitoring/follow-up. Participates in interdisciplinary treatment planning, crisis intervention, and coordination of services.

Requirements

• High school diploma or equivalent
• Certified Peer Support Specialist
• Community Health Worker (CHW)
• Community Navigator
• Parent/guardian of an individual with an I/DD or a TBI or a behavioral health condition
• Lived experience with an I/DD or a TBI or a behavioral health condition
• 2 years of paid care management experience
