Job Description
The Family Care-Service Coordinator position enables associates to primarily operate in the field, traveling to client sites or designated locations as their role requires, with occasional office attendance for meetings or training. The role is responsible for conducting service coordination functions for a defined caseload of individuals in specialized programs, including conducting assessments, developing Person Centered Support Plans, and managing individuals' physical health, behavioral health, and LTSS needs.
Requirements
- BA/BS degree
- Minimum of 2 years of experience working with a social work agency
- Ability to use technology
- Comfortable with traveling to worksite and other locations
