Evergreen Nephrology

3 years of experience in healthcare coordination, team-based approach to care, excellent community resources knowledge, and strong communication and teaching skills

5+ years of experience in nephrology, dialysis, or chronic disease management, with a BSN degree and active RN licensure

3-5 years of experience in instructional design, intermediate skills with Articulate 360, strong visual design skills, and ability to create job-realistic learning scenarios

LPN or LVN license, 1-2 years of clinical experience, strong clinical assessment and communication skills

1-2 years of experience in quality improvement, patient-facing outreach, understanding of quality workflows, and strong communication skills

Licensed clinician with experience in direct patient care and clinical operations, and experience leading clinical operations in value-based care, provider enablement, population health, or complex chronic care models
