Job Description
Join Family Connections as a Care Manager and make a difference in the lives of families. Provide trauma-informed and strengths-based support, conduct outreach, and coordinate housing intake processes. Work with children and families impacted by trauma and involved in child welfare.
Requirements
- Bachelor's degree in Human Services or Social Work
- At least two years of experience working with children and families
- Knowledge of community resources, social services, and mental health systems
- Strong crisis intervention skills
- Excellent communication and organizational skills
