Job Description
The Diabetes Community Care Coordinator (DCCC) will provide individualized diabetes self-management education to patients within the assigned clinic(s). The role involves active participation in patient care, coordination with necessary staff, and utilizing community resources. Bilingual English/Spanish communication is required. The DCCC will increase salary to $19.50 - $21.25 DOE once in-house certification is completed.
