Job Description
The Care Coordination Supervisor will provide oversight of care coordination services, lead a team of passionate employees, and guide client advocates in treatment plan development and review meetings. The role involves supervising a team of service providers, developing work schedules, and monitoring staff performance and productivity.
Requirements
- GED/HSD with a combination of a minimum of 5 years of field experience (preferably in community behavioral health), life experience, or relevant education
- OR Bachelor's Degree in Social Work/Psychology with an LSW/LPC or higher
- Valid driver's license and the ability to maintain auto insurance
- CPR/First Aid Certification or willingness to obtain one
- Willingness to submit background check, drug screening, and fingerprinting
- Ability to regularly use various computer apps including Microsoft Office, our HRIS, EHR, and other online systems
