FamilyWise is hiring a Wraparound Care Coordinator to facilitate a team process to give families voice and choice, with the goal of helping young people with behavioral health issues to live in a stable family or community setting.
Requirements
- Engaging children and families involved with multiple systems.
- Developing and facilitating family-centered teams.
- Conducting strengths and needs assessments.
- Developing and implementing individualized Wraparound plans.
- Collaborating with community partners.
- Modeling appropriate social skills for parents and their children.
- Communicating effectively with county workers.
- Supporting parents to improve life skills to be able to manage new needs as they arise.
- Participating in training, credentialing, and evaluation processes for High Fidelity Wraparound.
- Participating in monthly group coaching meetings and case consultations.
- Maintaining client files to include assessments, program plans, client progress, and assessments by other professionals, intake forms, client outcome data, referrals, and other pertinent information.
