Community Health Worker

Community Health Worker will work with specific target populations to identify and address Social Determinants of Health. The CHW will provide patient support, care coordination, and specialized assistance at Fenway Health clinical locations.

Requirements

• High School Diploma
• Knowledge of MS Office suite, particularly Excel and Outlook
• Strong organizational and attention to detail skills
• Ability to handle multiple tasks and prioritize assigned duties
• Ability to work harmoniously with diverse groups of individuals required
• Demonstrated ability to work respectfully and effectively with individuals from a wide range of backgrounds, perspectives, and life experiences
