MDS Nurse RN or LPN

We are seeking a dedicated MDS Nurse (RN or LPN) to join our team at Generations at Applewood in Matteson, IL. The successful candidate will have experience in long-term care and be passionate about providing excellent patient care.

Requirements

• Current RN or LPN licensed from the State of Illinois
• American Association of Nurse Assessment Coordination (AANAC) certification preferred
• RAC-T Certified preferred
• Knowledge of Minimum Data Set (MDS), ICD-10 Coding
• Two (2) to three (3) years of applicable experience as a MDS Nurse
• Must be CPR Certified
• Experience in a long term care setting
