Certified Diabetes Care and Education Specialist (Modesto Women's)

The Certified Diabetes Care and Education Specialist provides individualized diabetes self-management education and training to individuals and groups. This position will work at our Modesto Women’s Clinic, Monday – Friday, 8:00am – 5:00pm.

Requirements

• Knowledge of community health, nutrition, motivational interviewing, and adult learning.
• Excellent customer services and interpersonal skills.
• Working knowledge of Microsoft Office Suite.
• Written, presentation and training skills.
• Ability to analyze data and compile reports.
• Meet deadlines and prioritize tasks.
• Work independently.
• Effectively interact with community agencies, patients, and clinical personnel.
• Ability to augment schedule to meet patient access needs.
