Housing Case Manager/ Specialist

Grace Community Care & Homes delivers community-based housing support services for adults with disabilities, behavioral health needs, and other vulnerable populations. As a Housing Specialist, you will guide clients through the housing process, build relationships with landlords, and provide ongoing tenancy support.

Requirements

• Bachelor's degree in social work, human services, public health, or related field preferred
• Minimum 2 years of experience in case management, housing navigation, or social services
• Experience working with vulnerable populations preferred
• Knowledge of affordable and supportive housing systems is a plus
• Strong organizational, communication, and problem-solving skills
• Ability to manage a 30-45 client caseload in a field-based environment
• Proficiency with Microsoft Office and electronic documentation systems
• Reliable transportation and ability to travel throughout the Bronx for client visits and housing meetings
