Provide medication management services to all patients of Health Sciences North (HSN). Evaluate the appropriateness and effectiveness of patients’ medications, drug information and interdisciplinary drug therapy management support, and drug distribution services to both inpatients and outpatients at HSN.
Requirements
- Perform assessments of all medication therapies prescribed for HSN patients.
- Provide oral and written follow-up with treatment recommendations to the prescriber as required.
- Provide medication reconciliation services and interact with patients/families and other health care professionals with regards to patients’ home medication usage.
- Review medication orders for all HSN patients and ensure safe and timely provision of medication distribution.
- Ensure completion of required documentation in conformance with the standards of practice of the Ontario College of Pharmacists (OCP) and HSN policies.
- Provide direct medication-related care as delegated by policy approval through the Medical Advisory Committee (e.g. Pharmacy and Therapeutics Committee) or by direct delegation of the Physician.
- Interact with health care professionals and the patient/family regarding the patient’s plan of care and how to deal with a drug-related problem.
- Oversee drug use control of all medications, including security of narcotics and controlled drugs.
- Maintain compliance with the Controlled Act of Prescribing, Dispensing, and Selling under the Regulated Health Professionals Act (RHPA).
- Ensure verification of all technical work by directly confirming or via the standards of practice of the OCP and HSN policies.
- Participate in the development of operational policies and procedures for the Pharmacy and protocols for the delegation of the Controlled Act of Prescribing, Dispensing, and Selling.
- Perform Therapeutic Drug Monitoring for high risk drugs.
- Provide drug therapy management education for physicians, nurses, allied health care providers, patients and caregivers.
- Liaise with community pharmacists to optimize seamless patient care at admission and discharge.
- Perform and be an advocate for adverse drug reaction and medication error reporting and meet with interdisciplinary groups specific to patient care areas or organization wide to implement preventative changes for the future.
- Complete departmental projects (e.g. formulary reviews, IV monographs, DUE projects, policy and procedure development, research projects).
- Determine and align improvement projects with HSN’s Strategic Plan; monitor and adjust to achieve goal outcomes.
- Participate in education and training specific to current, relevant federal and provincial health and safety legislation, standards and guidelines.
- Educate and promote health, safety and wellness in the work place.
- Represent the department or program on various committees and in meetings as required.
- Perform other duties as required.
