Job Description
Ontario Health at Home is seeking an experienced Care Coordinator to assess and determine patient care needs, provide referrals, and engage with patients and healthcare professionals. This role involves a lead role in providing connected, accessible, patient-centred care and supporting their professional growth and well-being, and will benefit from a collaborative team and various resources. The role is highly adaptable, with potential for work in office, community, or a healthcare facility.
Requirements
- Membership in good standing with relevant regulatory bodies (College of Nurses, Physiotherapists, Occupational Therapists, Social Workers, Dietitians, Audiologists, and Speech Language Pathologists)
- 2+ years of recent experience in community health or a related field
- Knowledge of the health care delivery system and community resources
- Excellent interpersonal, communication, assessment, problem-solving, and decision-making skills
- Effective time management, prioritization, and organizational skills
- Valid driver’s license and access to a reliable vehicle
- Proficiency in a Windows environment
