Job Description
Care Coordinator role involves determining patient eligibility and care needs, ensuring access to community services, and collaborating with patients, caregivers, and healthcare professionals. Requirements
- Current member in good standing with the applicable regulatory body: Ontario College of Nurses, College of Physiotherapists of Ontario, College of Occupational Therapists of Ontario, Ontario Association of Social Workers
- Recent experience in community health or a related field
- Knowledge of the healthcare system and community resources
- Excellent interpersonal, communication, evaluation, problem-solving, and decision-making skills
- Time management, prioritization, and organizational skills
- Valid driver's license and access to a reliable vehicle
- Mastery of Windows environment
- Advanced proficiency in English
