Job Description
Care Coordinator position with Home Care Ontario South-West, responsible for determining patient eligibility, ensuring access to community services, and collaborating with patients, caregivers, and healthcare professionals.
Requirements
- Member in good standing of the relevant regulatory organization (Registered Nurses Association of Ontario, College of Physiotherapists of Ontario, College of Occupational Therapists of Ontario, Ontario Association of Social Workers, Dietitians of Ontario, Speech-Language and Audiology Canada)
- At least two years of experience in community healthcare or a related field
- Knowledge of the healthcare system and community resources
- Valid driver's license and access to a reliable vehicle
- Windows environment
