Coordonnateur de soins

We are seeking a highly skilled and experienced community care coordinator to join our team. As a community care coordinator, you will determine patient eligibility and their care needs, ensure access to community services and direct patients to those services as needed, and collaborate with patients, caregivers and healthcare professionals.

Requirements

• Current member in good standing with the applicable regulatory organization
• Order of Nurses and Nurse Practitioners of Ontario
• Order of Physiotherapists of Ontario
• Order of Occupational Therapists of Ontario
• Order of Social Workers and Social Service Workers of Ontario
• Order of Dietitians of Ontario
• Order of Audiologists and Speech-Language Pathologists of Ontario
• Minimum two years of experience in community health care or a related field
• Knowledge of the healthcare system and community resources
• Excellent interpersonal, communication, evaluation, problem-solving and decision-making skills
• Time management, prioritization, and organizational skills, with the ability to work independently and collaboratively in a fast-paced multidisciplinary environment
• Established ability to complete accurate documentation, reports and forms
• Valid driver's license and access to a reliable vehicle
• Proficiency in the Windows environment
