Job Description
We are seeking a Primary Care Physician to join our growing team of passionate, mission-driven clinicians. The successful candidate will evaluate patients' acute and chronic conditions, manage their medications, and provide empathetic patient education. The PCP will work closely with fellow clinical teammates to provide comprehensive primary care and help achieve key organizational objectives.
Requirements
- Licensed MD/DO with board certification in Family Medicine (ABFM) or Internal Medicine (ABIM)
- Experience managing complex diseased longitudinally over time
- Understanding or willingness to learn how to practice in a value-based care (VBC) model
- Experience managing overall patient care and devising individualized care plans
- Strong problem-solving and critical thinking skills
- Up-to-date knowledge of medical advancements and technologies
