Job Description
Herself Health is seeking a Primary Care Physician to join their interdisciplinary care team. The PCP will evaluate patients' acute and chronic conditions, manage medications, mental health, and provide empathetic patient education. The role involves working closely with fellow clinicians to provide comprehensive primary care and assisting with organizational objectives and metrics.
Requirements
- Licensed MD/DO with board certification in Family Medicine (ABFM) or Internal Medicine (ABIM)
- Experience managing complex diseases longitudinally over time
- Understanding or willingness to learn value-based care (VBC) model
- Experience managing overall patient care and devising individualized care plans
- Strong problem-solving and critical thinking skills
