Job Description
The Care Manager II provides community-based services to youth and adults in Livingston, Genesee, and Orleans counties, working as part of a team to reduce emergency room visits, hospitalizations, and improve Medicaid costs. This role involves comprehensive assessments, care planning, linkage to services, and advocacy. The position is full-time, 40 hours a week with a flexible schedule and potential for hybrid work.
