RN, Director of Case Management

We are looking for a dynamic, experienced, independent, and results-driven RN, Director of Case Management to join our team. If you thrive in a fast-paced environment and excel at leading case management strategies that drive client success, we want you on our team!

Requirements

• Active and unencumbered KY registered nurse license
• Minimum of a bachelor’s degree in social work, nursing, healthcare management, or a related field
• At least five years of progressive leadership experience in case management, preferably in healthcare, social services, or nonprofit settings
• Proven track record of managing complex caseloads and exceeding performance targets
• Deep understanding of regulatory requirements, including HIPAA, Medicaid, and other relevant standards
• Exceptional interpersonal and communication skills with the ability to build trust and collaboration across teams and stakeholders
• Demonstrated ability to drive results in a fast-paced, mission-driven environment
• Strong organizational skills with an emphasis on detail, problem-solving, and multitasking
• Proficiency in case management software and tools
• Certification in Case Management (CCM or equivalent) is a plus
